GUIDE is designed for people living with dementia and their family members. When support is fragmented, caregivers burn out, and avoidable crises happen. A clear referral pathway helps families get relief sooner.

What GUIDE May Provide for Eligible Patients

  • Up to $2,500/year for respite/in-home care support (renews annually if eligibility continues)
  • Respite care hours are paid by Medicare so caregivers can rest, work, or attend appointments
  • A dedicated dementia care navigator for coordination and community resources
  • Caregiver training and support to reduce stress and improve safety

Quick Eligibility Checklist

Eligible folks may include those who:

  • Have Traditional Medicare
  • Have suspected or confirmed dementia
  • Live at home or in the community (including ALF/ILF)
Not eligible if the person:
  • Has Medicare Advantage
  • Is in certain SNP plans
  • Lives in a nursing home/long-term care
  • Is in Hospice or PACE
